One-liner#

Monitoring requirements are a common cause of adverse events and denials—standardize them.

High-yield categories to track#

  • Antihypertensives/diuretics (electrolytes/renal function as appropriate)
  • Diabetes meds (A1c cadence; renal function for some agents)
  • Statins (baseline and symptom-driven monitoring per policy)
  • Anticoagulants/antiplatelets (indication, renal dosing when relevant, bleeding risk)
  • NSAIDs (renal/GI/CV risk; avoid chronic use in high-risk patients)

Documentation checklist#

  • Indication and intended duration (especially for “bridge” meds)
  • Monitoring plan and target parameters
  • Safety counseling (side effects, red flags, when to stop/seek care)
