Minimum requirements (suggested)#

  • Active patient relationship + medication list reconciliation
  • Last visit date and next follow-up due date
  • Indication is documented somewhere in the chart (problem list or A/P)
  • Monitoring is up to date (labs/vitals) for meds that require it

Refill decision checklist#

  • Confirm dose, route, frequency, and adherence
  • Check interactions and duplicate therapy
  • Confirm recent vitals/labs relevant to the medication
  • Provide an interval-limited refill if overdue for monitoring, with a follow-up plan
