Minimum requirements (suggested)#
- Active patient relationship + medication list reconciliation
- Last visit date and next follow-up due date
- Indication is documented somewhere in the chart (problem list or A/P)
- Monitoring is up to date (labs/vitals) for meds that require it
Refill decision checklist#
- Confirm dose, route, frequency, and adherence
- Check interactions and duplicate therapy
- Confirm recent vitals/labs relevant to the medication
- Provide an interval-limited refill if overdue for monitoring, with a follow-up plan
