• Pigmented lesion with melanoma features or uncertain diagnosis (do not freeze; evaluate/biopsy or refer)
• Lesions suspicious for malignancy (biopsy first)

• Diagnosis/indication and number of lesions
• Anatomic location(s)
• Consent and expected outcomes (blistering, hypopigmentation, scarring, recurrence)
• Technique summary (freeze/thaw cycles, if you track it)
• Aftercare instructions
