- Biopsy type (shave/punch/incisional), tool size (e.g., punch diameter)
- Consent and risks (bleeding, infection, scar, incomplete diagnosis)
- Local anesthesia (agent and amount)
- Hemostasis method
- Specimen labeling and what was sent to pathology
- Aftercare instructions and result follow-up plan
